Healthcare Assistant - District Nursing
Job summary
An exciting opportunity has arisen for a Band 3 Health Care Assistant to join the Community Nursing Team based at SohoCentre forHealthand Care.
This rewarding role offers the opportunity to support people within their own homes, helping them tomaintaintheir independence, manage long-term conditions and achieve the best possible health outcomes. Working as part of a multidisciplinary district nursing team, you will contribute to the delivery of high-quality, person-centred care to a diverse patient population.
We are looking for a Health Care Assistant who holds an NVQ Level 3 in Health and Social Care and has experience working in a hospital or community healthcare setting. Experience of working within the NHS is essential, andpreviouscommunity experience would beadvantageous.
This is an excellent opportunity for someone who is committed to delivering compassionate care and would like to further develop their skills within a supportive community nursing service. Main duties of the job
- Provide nursing skills such asadministration ofmedication, wound care, catheter care,supporting patients at end of lifeetc.
- Provide high-quality, person-centred care to patients in their own homes.
- Support individuals with long-term conditions to maintaintheir independence,wellbeingand confidence.
- Work collaboratively with registered nurses and members of the multidisciplinary team to deliver safe, effectiveand responsive care.
- Assistinmaintaininghigh standardsof clinical practice and service delivery.
- Develop your knowledge and skills through training, supervisionand professional development opportunities.
- Contribute to a positive and inclusive team environment that values diversity and respects the needs of the local community.

Experience
Essential
- Experience of working in a health or social care setting
- Experience of working as part of a team
- Experience with the use of Microsoft Office and a variety of databases.
Desirable
- Experience with the NHS patient information database (Rio, CIS)
Skills & Knowledge
Essential
- Knowledge of NHS
- Demonstrate an acceptable level of numeracy and literacy
- An awareness of risk management.
- Knowledge of multi-agency services available and processes for referral
- Knowledge of the effects of the ageing process and needs of vulnerable adults in relation to their health and well-being
Desirable
- Knowledge of local health needs strategies.
Education/Qualification
Essential
- Higher Professional Diploma in Health and Wellbeing (NVQ/QCF - Level 3) or A recognised equivalent course
- Willingness to undertake further training as appropriate
Desirable
- GCSE grade A-C or equivalent in English and Maths ECDL certificate
- Registration with the national regulatory body (when it becomes available).
